Public Records: Restaurant Inspections

The following are restaurant inspections with critical violations, as conducted by the Zanesville-Muskingum County Health Department between Sept. 1-30

Sept. 13

Circle K, 60 W. Main St., New Concord, Facility did not have all 13 reportable illnesses included in the company’s sick policy. Observed dented can in the facility. Mold buildup on the ice dispenser of soda machine. Cold brew coffee machine had a temperature of 44.2 degrees. Bathroom cleaner, break fluid, spray paint and windshield fluid stored over food and single-use items. Ingredients list for doughnuts not displayed on self-service unit.

Sept. 14

Domino’s Pizza, 2355 Maple Ave., Food left uncovered on prep line in front of the kitchen.

Par Mar Store, 4950 East Pike, Mold buildup on slushee machines and ice dispenser of soda machine. Buildup of ice on food product in walk-in cooler. Affected food discarded during inspection. Food in walk-in cooler past discard date, other food not date marked. Affected food discarded during inspection. Toxic chemicals stores above food products.

Sept. 15

BP, 1305 Maple Ave., Raw bacon stored above ready to eat foods in walk-in cooler.

Scott’s Diner, 41 E. Main St., New Concord, Food debris buildup on deli slicer.

Fuel Mart, 201 Main St., New Concord, Bottled drinks stores in handwashing sink. Mold buildup on soda machine.

Sept. 19

Clay Gardens Place, 3784 Frazeysburg Road, Ham not properly date marked.

Kentucky Fried Chicken, 2160 Maple Ave., Mold buildup on soda machine.

Philo High School, 4000 Millers Lane, Duncan Falls, Internal temperature of TCS foods was 45 degrees. Food to be discarded.

Cracker Barrel, 225 Scenic Crest, Debris buildup on can opener. Pot pie mix and mac and cheese not properly date marked.

Dillon Onestop, 6325 Newark Road, Nashport, Employee present during inspection had minimal knowledge of good code and could not list proper 3 compartment sink set up, proper cold holding temp or proper date marking procedures. Section of cooler was 50 degrees during inspection. Person in charge was not permitted to discard food so TCS foods were pulled from display and placed on a cart for manager to discard. No date marking observed. TCS foods older than 7 days were discarded during inspection.

Sept. 20

Horse Shoe Bar & Grill, Raw bacon stored above ready to eat foods in cooler. Bleach stores above single-use articles.

El Parian, 145 N. Maysville Ave., Trash can roller stored on top of open box of potatoes. Walk-in cooler at 48 degrees. All TCS foods above 45 degrees discarded, remaining food to be put on ice or in different units to hold at 41 degrees or below.

Sept. 26

Four Star Pizza, 1536 Maysville Pike, Food equipment stores in handwashing sink. Bleach and all purpose spray bottles and Lysol stored over food and gloves.

Sept. 28

Continuing Healthcare Beckett House, 1280 Friendship Drive., New Concord, Chemicals stored over gloves.

Garland Hotel, 150 Garland Way, New Concord, Utensils stored in handwashing sink. Mold buildup on milk dispensing machine.

Wally’s Pizza and Subs, 15 W. Main St., New Concord, Glass cleaner stored above food.
