My guess is that the DEF in question had a waiver request submitted for them, but the person that made that request had an earlier request processed and the player he was gonna drop to pick up the DEF was used to drop the earlier request. Since the player was no longer on the team, that waiver request was not processed and no other team made a claim on the DEF.

Happens every year. Yahoo's default schedule is always to play every other team in the league once and then repeating the same schedule until the end of the season. For example, if you look at your schedule (10 team league) your week 10-13 schedule most likely is the same as your weeks 1-4.
Before the season starts, the commissioner has the opportunity to change the schedule to fit their leagues requirements. there's also a "regular season matchup matrix" at the bottom that list teams by divisions and the number of games played against every other team in the league. This comes in handy when making a balanced schedule. I suggest next year using a schedule generator to come up with a schedule and having the commissioner adjust the yahoo default schedule to the new generated on. Footballguys has 157 default schedules to choose from that will handle about every conceivable scenario.

Yahoo doesn't support custom playoffs scheduling. This question should have been brought to the attention of your league when it was set up as ALL 3 options for playoff seeding include some form of the following statement: "All other playoff teams will be determined by overall standings regardless of division results."

That's what I'm thinking, Involves a player playing tonight against him.
However, if the trade didn't go through before the game tonight, it wouldn't be processed until next Tuesday and may go against the original reasons behind the 2 trade owners making the trade.
If he wants the power to decide if a trade should be processed (which 99.99% of all trades should), then it should be done ASAP

Yes, there's a way to put a 1 or 2 day hold before the trade takes affect. Don't approve the trade for 1 or 2 days.
But WHY would you want a hold before the changes take place? You've already decided to be the one to approve/disapprove a trade, so why should there be a waiting period on your decision going through?

You can only add a player to your IR slot if he is actually on IR in real life. Since he is just listed as out, you need to keep him on your bench. There is no setting the commissioner can change to get around this.
